I’m exploring Home Help

Start with a short service inquiry. Our team can explain the referral process and point you toward MDHHS.

I already have Medicaid or Home Help

Tell us whether you need to begin Home Help or want to discuss selecting Yes You Can as your agency.

From first contact to a care plan

You don’t have to figure it out alone.

01

Contact Yes You Can

Tell us whether you are exploring care or already receive Home Help.

02

Review your next steps

We’ll discuss your inquiry, available support, and the appropriate paperwork.

03

Assessment & authorization

MDHHS determines Home Help eligibility and authorizes covered services.

04

Plan your care

Coordinate the approved support and caregiver arrangements before care begins.

Submitting an inquiry does not guarantee Medicaid eligibility, Home Help approval, or service authorization.

Know what comes next

One step at a time.

The first inquiry asks only for the information needed to contact you and understand your general request. Official applications, certification, and supporting records remain separate.

Already receiving Home Help?

Tell your MDHHS adult services worker about your provider choice. Agency payment authorization requires state coordination.

No. The Get Started form is a service inquiry to Yes You Can. MDHHS determines eligibility and service authorization. An inquiry does not guarantee approval or care.

Tell our team that you already receive Home Help. Our team can discuss provider selection and the steps that must be coordinated with your MDHHS adult services worker.

Visit the external Michigan enrollment website through our Check Medicaid Status page, or contact our team for help finding the appropriate resource. Please do not include Medicaid identification numbers in a general website message.

Some relatives and friends may qualify, subject to current state enrollment and screening requirements. The Family Caregiver page links to official guidance; payment and approval are not guaranteed.
